Registering with the surgery
Please note that the registration process can take up to 5 days.
In order to register as a patient at Weston House Surgery and Hatherley Branch Surgery, please visit in person, bringing with you photographic proof of ID (e.g. passport or driving licence) and a utility bill or similar with your address on it. We will ask you to complete our registration form (PDF copies of which can be found listed below).
To speed up the process, please complete the forms below and bring with you to the surgery, along with your photographic proof of ID (e.g. passport or driving licence) and a utility bill or similar with your address on it:
- registration form for adults (aged over 16) (PDF)
- registration form for aged under 16 (PDF)
- registration form for babies (new born children) (PDF)
Please also fill out the GMS1 form (PDF) to register.
You can also register with a GP surgery online: Westonhouse Practice.
Please be aware you will still need to bring your ID into the practice if you would like to register for online services.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
