Surgery Closure on Tuesday 14th July

Hatherley Branch Surgery will be closed all day on Tuesday 14th July so that essential maintenance can be carried out on the property.

Weston House Practice and the phone lines will remain open from 8:00-6:30pm, please still continue to contact the Practice should you need medical assistance.

As a reminder your local Pharmacy can deal with the following conditions with the ability to prescribe antibiotics without the need for a GP appointment:

> Earache (aged 1-17 years)

> Impetigo (aged 1+)

> Infected insect bites (aged 1+)

> Shingles (aged 18+)

> Sinusitis (aged 12+)

> Sore throat (aged 5+)

> UTI (Women aged 16-64)

In a life threatening emergency please call 999.

Registering with the surgery

Please note that the registration process can take up to 5 days.

In order to register as a patient at Weston House Surgery and Hatherley Branch Surgery, please visit in person, bringing with you photographic proof of ID (e.g. passport or driving licence) and a utility bill or similar with your address on it. We will ask you to complete our registration form (PDF copies of which can be found listed below).

To speed up the process, please complete the forms below and bring with you to the surgery, along with your photographic proof of ID (e.g. passport or driving licence) and a utility bill or similar with your address on it:

Please be aware you will still need to bring your ID into the practice if you would like to register for online services.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
