Medical report request

Use this service to request a medical report.

For Subject Access Request (SARS) please allow 28 days, this is a free NHS service. For all non-NHS requests e.g. insurance forms these can take 6-8 weeks and is subject to a fee which will be advised at the point of the request. Payment must be received before the work is started.

  • are registered at the surgery

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
