Travel risk assessment

Use this service to arrange travel vaccinations and get advice for your upcoming trip abroad if you are a registered patient at the Practice.

To help the Nurses assess your requirements you need to complete a Travel Questionnaire detailing when and where you are planning to visit.

Please note this questionnaire needs to be completed and returned to the Practice at least 8 weeks before your travel date, anything less than that, we may not be able to accommodate your vaccines as we need at to vaccinate at least 6 weeks prior to travel and our clinics are currently booked weeks in advance.

If you are travelling to more than 2 countries you need to visit a travel clinic and the return to the Practice to have your NHS vaccines, provided there is enough time.

If you have any questions, please contact the Surgery on 01242 215000 to speak to a Care Navigator.
